Stages and Requirements for Community Health Centers

Community Health Centers (Puskesmas) that intend to submit proposals for an initial accreditation survey and for a reaccreditation survey must meet several mandatory requirements
Proposals for the initial accreditation survey must meet the following mandatory requirements:
- Holds a business license and is registered with the Ministry of Health.
- The head of a community health center must have a minimum educational background of a Bachelor’s degree in Health. For community health centers in remote and very remote areas where no one with a Bachelor’s degree in Health is available, the position may be filled by a functional health worker with a minimum educational level of an Associate’s degree.
- The community health center director has completed training in community health center management.
- There is evidence that the Medical Equipment and Infrastructure Application (ASPAK) has been 100% updated and 100% validated by the Regency/City Health Office.
- All medical and healthcare personnel at the Community Health Center (Puskesmas) hold a Certificate of Registration (STR), as verified through the Health Human Resources Information System (SISDMK). �
- At least 80% of the medical staff at community health centers (Puskesmas) hold a valid License to Practice (SIP), as verified through the Health Human Resources Information System (SISDMK). �
- There is evidence of reporting on the National Quality Indicators (INM) through the Health Care Facility Quality app for the past 12 (twelve) months.
- There is evidence of Patient Safety Incident (PSI) reporting via the Healthcare Facility Quality application for the past 12 (twelve) months.
Proposals for re-accreditation surveys must meet the following mandatory requirements:
- Holds a business license and is registered with the Ministry of Health.
- Community health centers must have doctors.
- The head of a community health center must have a minimum educational background of a Bachelor’s degree in Health. For community health centers in remote and very remote areas where no one with a Bachelor’s degree in Health is available, the position may be filled by a functional health worker with a minimum educational level of an Associate’s degree.
- The community health center director has completed training in community health center management. There is evidence that the Health Facilities, Infrastructure, and Equipment Application (ASPAK) has been 100% updated and 100% validated by the Regency/City Health Office, with at least 60% compliance with SPA requirements, and 100% validated by the Regency/City Health Office.
- All medical and healthcare personnel at the Community Health Center (Puskesmas) hold a Certificate of Registration (STR), as verified through the Health Human Resources Information System (SISDMK). �
- At least 80% of the medical staff at community health centers (Puskesmas) hold a valid License to Practice (SIP), as verified through the Health Human Resources Information System (SISDMK).
- There is evidence of reporting on the National Quality Indicators (INM) through the Health Care Facility Quality app for the past 12 (twelve) months.
- There is evidence of Patient Safety Incident (PSI) reporting via the Healthcare Facility Quality application for the past 12 (twelve) months.

Clinics that intend to submit a proposal for an accreditation survey must meet several mandatory requirements
Proposals for the initial accreditation survey must meet the following mandatory requirements:
- Holds a business license and is registered with the Ministry of Health.
- The person in charge of technical matters at the clinic is a medical professional who holds a License to Practice (SIP) at that clinic.
- All medical and healthcare personnel at clinics that provide healthcare services (care providers) must hold valid Certificates of Registration (STR) and Licenses to Practice (SIP), as verified through the Health Human Resources Information System (SISDMK).
- There is evidence that the Medical Equipment and Infrastructure Application (ASPAK) has been 100% completed and 100% validated by the Regency/City Health Office.
- There is evidence of reporting on the National Quality Indicators (INM) through the Health Care Facility Quality app for the past 3 (three) months for the initial survey and the past 12 (twelve) months for the re-accreditation survey.
- There is evidence of Patient Safety Incident (PSI) reporting via the Healthcare Facility Quality application for the past 3 (three) months for the initial survey and the past 12 (twelve) months for the re-accreditation survey.

Stages and Requirements for Health Laboratories and Blood Transfusion Units

Health laboratories and blood transfusion units that intend to submit a proposal for an accreditation survey must meet several mandatory requirements
Proposals for the initial accreditation survey must meet the following mandatory requirements:
- Hold a valid business license and be registered with the Ministry of Health.
- There is evidence that the Medical Equipment Infrastructure Application (ASPAK) has been 100% updated.
- All medical and health personnel at Health Laboratories and Blood Transfusion Units that provide health services (care providers) must hold valid Registration Certificates (STR) and Practice Licenses (SIP), as verified through the Health Human Resources Information System (SISDMK).
- All medical personnel at Health Laboratories and Blood Transfusion Units that provide health services (healthcare providers) must hold a valid Certificate of Registration (STR) and a valid Certificate of Practice (SIP), or a letter of assignment in accordance with applicable laws and regulations; meanwhile, other health personnel must hold a Certificate of Registration (STR).
- There is evidence of reporting on the National Quality Indicators (INM) through the Health Care Facility Quality app for the past 3 (three) months.
- There is evidence of Patient Safety Incident (PSI) reporting via the Healthcare Facility Quality app for the past 3 (three) months.
