Surveyor Qualifications
Decision of the Director General of Health Services NUMBER HK.02.02/I/3991/2022
General Criteria
- Indonesian citizen;
- In good health, as evidenced by a Medical Certificate from a doctor and a Certificate of No History of Drug Use, so as to be able to perform the duties of an accreditation surveyor;
- Has never been involved in a criminal offense or committed a criminal offense as determined by a court ruling that has become final and binding, as evidenced by a certificate of no criminal record;
- Willing to be assigned to conduct surveys in any region, provided that a signed and stamped statement is attached.
Specific Criteria
a. Community Health Center
- Resource Management and SMEs Division
- Medical personnel or other healthcare workers with at least a bachelor’s degree (S1) in a health-related field; and
- Have at least 3 (three) years of experience working at a community health center and/or clinic, managing primary health care programs, and/or managing primary health care quality programs.
- Service Management and Support Division
- Medical personnel; and
- Have at least 3 (three) years of work experience at a community health center and/or clinic.
b. Clinic
- Resource Management and SMEs Division
- Medical professionals and/or other healthcare workers with at least a bachelor’s degree (S1) in a health-related field; and
- Have at least 3 (three) years of experience working at a community health center and/or clinic, managing primary health care programs, and/or managing primary health care quality programs.
- Service Management and Support Division
- Medical personnel; and
- Have at least 3 (three) years of work experience at a community health center and/or clinic.
c. Health Laboratory and Blood Transfusion Unit
- Healthcare Management
- Medical or healthcare professionals with at least a master’s degree (S2) in a health-related field and a bachelor’s degree (S1) in a health-related field; and
- Have experience:
a. Managing a health laboratory or blood transfusion unit (UTD); and/or
b. Managing quality and accreditation programs for health laboratories or blood transfusion units (UTD), or other healthcare facilities, for a minimum of 3 (three) years.
- Technical service division
- Medical personnel with a minimum of a professional degree in laboratory medicine, or health care personnel with a Bachelor’s degree (S1) or a Level 4 Diploma (D IV) in a field related to clinical laboratory science or blood transfusion services; and
- Have at least 3 (three) years of work experience in a Health Laboratory or Blood Transfusion Unit (UTD) as a technical manager of a Health Laboratory or UTD.
